6 months temporary position possibly leading to permanent dependant on success of project

Job Description

As part of our ongoing commitment to improve the discharge process for patients transferring to Residential and Nursing Homes following a stay in acute hospitals, Nottinghamshire Care Association are expanding, and we are looking to appoint a temporary Care Home Trusted Assessor based at Sherwood Forest Hospitals Trust and a Nursing Home.

You will support residents primarily in Sherwood Forest Hospitals Trust as part of an integrated team of Health and Social Care Staff, and will liaise with local residential and nursing homes, including visiting them. As part of the move towards a 7-day working week in health and care, members of the team will also be required to provide some weekend cover.

The primary responsibility of the Trusted Care Home Assessor is to act as an independent and unbiased assessor on behalf of care homes in relation to undertaking assessments, beyond the statutory, that need to meet the needs of the individual. An important part of the position will involve liaising with the care homes to obtain information about the patient and facilitate flow of information between the hospital, the home, Adult Social Care and the Discharge team. Other responsibilities will include recording information, so it can be decided if the project should be extended to a permanent position, liaising with Transport and pharmacy to aid discharge and attending the project meetings.

The ideal candidate will have:

·         Experience of assessing Residents/Patients for discharge to care Homes

·         Nursing experience

·         excellent communication and influencing skills

·         A full Driving Licence and access to personal transport

·         Experience of Care Homes Management

·         To be resilient and self-reliant with a passion for placing the individual at the centre of decisions about care
